Why the DHS-390 gets confused with caregiver enrollment
Home Help has paperwork for the person receiving services and separate enrollment for the person providing services. The DHS-390 belongs on the Adult Services/client side. CHAMPS belongs on the provider/caregiver side.
That difference is important. Completing caregiver enrollment does not replace the client’s application and assessment. Likewise, sending a DHS-390 does not make the caregiver ready for payment. The two tracks need to meet at the approved Home Help case.
