MI Coordinated Health: home care through HIDE SNP plans in Michigan

If you have both Medicare and Medicaid, your health plan may cover personal care and other long-term support at home. MI Coordinated Health (MICH) is Michigan's coordinated Medicare and Medicaid coverage. Members receive MICH through a participating insurance company's HIDE SNP plan. That insurance plan handles care coordination, its provider network, covered benefits and authorizations.

How the coverage path works

Enrollment determines who manages Medicaid personal care.

Traditional Michigan Home Help is the state-administered Medicaid path outside MICH enrollment. After an eligible person enrolls in MICH, covered long-term services and supports and personal care are managed through the member's HIDE SNP under the plan's network and authorization rules.

Outside MICH enrollment

Traditional Michigan Home Help

For Medicaid beneficiaries receiving personal care through Michigan's state-administered Home Help program. This is separate from the MICH managed-care path.

After MICH enrollment

MICH coverage through the insurance plan

For eligible adults with both Medicare and Medicaid who enroll in the MICH program. A participating insurance company administers the member's Medicare and most Medicaid benefits through its HIDE SNP plan, including long-term services and supports and personal care services. Network and authorization rules apply.

Check counties and plans

The key distinction is enrollment. Having both Medicare and Medicaid can make a person eligible to enroll in MICH if the other program requirements are met. Once enrolled, the member's insurance company manages covered home-care benefits through its HIDE SNP plan under MICH.

Who can enroll

MICH program requirements

MDHHS says a person must meet all of these conditions:

Be age 21 or older
Be enrolled in both Medicare and Medicaid
Not live in a state-operated veterans home
Not be enrolled in hospice care
Live in a county where MICH is available

Have Aetna, Humana, Molina or UnitedHealthcare?

One member process, with coverage verified for your exact plan.

First confirm Medicare and full Medicaid eligibility and identify the member's participating HIDE SNP insurance plan. That plan provides the member's MICH coverage and handles the assessment for personal care services or the MICH HCBS waiver. Allcare is contracted with the four plans below and will verify the exact product, county, covered services and authorization before care begins.

01

Confirm Medicare and full Medicaid

Start by confirming the person has Medicare Parts A, B and D, full Medicaid, is age 21 or older and lives in a MICH service county. Other eligibility exclusions may apply.

02

Confirm the member’s MICH insurance plan

An eligible person receives MICH coverage by enrolling in a participating insurance company’s HIDE SNP plan. The member’s insurance plan is the main point of contact.

03

Ask the plan for a PCS or HCBS assessment

The insurance plan assesses the member for State Plan personal care services or, when applicable, the MICH HCBS waiver.

04

Begin authorized care with Allcare

After the insurance plan confirms covered services and authorization, Allcare can arrange the approved home-care services.

West Michigan transition

A smaller focus now, ahead of the 2027 expansion.

Allcare is preparing to support West Michigan families as MI Coordinated Health expands. This information is intentionally secondary to the counties where MICH is already active.

Active during 2026

Southwest Michigan

Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph and Van Buren counties are included in the current MICH coverage area. A member must still meet the MICH requirements and enroll in a participating plan.

Scheduled for January 1, 2027

Grand Rapids, Holland and the broader West Michigan region

MDHHS lists the West Michigan Prosperity Alliance in the 2027 MICH launch. The announced plans include Aetna, Humana, Molina and UnitedHealthcare, along with other participating plans. Until the expansion begins, this is transition information—not a promise of current MICH coverage.

Planning for the 2027 expansion? A West Michigan resident who has both Medicare and Medicaid may be able to enroll in MICH when the program becomes available in the county, if all program requirements are met. After MICH enrollment, covered LTSS and personal care are managed through the member's HIDE SNP.

What changed

From MI Health Link to MICH

MI Health Link ended on December 31, 2025. MI Coordinated Health began on January 1, 2026, in the counties listed above.

MDHHS says MICH is scheduled to expand statewide in 2027. Under MICH, a participating insurance company coordinates Medicare and most Medicaid benefits, including long-term services and supports, through its HIDE SNP plan.

For an enrolled MICH member, the health plan manages covered benefits and authorizations that fall under MICH, including long-term services and supports and personal care services.
